Problem:
Not able to import an Age, Gender and Rate columns premium rate table

Solution:

Column headers that are considered as qualifiers i.e any value that is defined as a Lookup Table Allowable column should not be considered as a Rate/Amount heading.
The last column in the imported table should be considered as the Rate/Amount heading and when the table allows two Rate/ Amount headings, then the last column should be considered as Rate/Amount heading 2 and the last but one column should be considered as Rate/Amount heading 1 and the column headers defaulted in the imported lookup table.
All other existing validations related to valid qualifiers as part of the imported table should remain
