[SICSR-15987] Abend in Rate / Amount Heading 2 not as per Look Up table Allowable Column Definition

Problem:

Abend when the Rate/ Amount field header is manually edited and changed from the defaulted value from the file when a table is imported

Solution: 

When a file is imported as a lookup table the values for the fields Rate/Amount Heading should be defaulted as per the imported file and the fields should be disabled for editing
The Rate/ Amount Heading fields should be available for input only when the table is defined
